This form is to be used by Medical Scientists, Psychologists and related designations in small, rural hospitals, stand alone CHCs and other hospitals not directly funded for Study Leave. Members employed by Health Services should contact their Human Resources Department to apply for Study Leave.
NB: When completing your form you should include any existing HQA allowances into your hourly rate.
